Pancreatic hyperenzymemia: clinical significance and diagnostic approach.

چکیده
An increase in the serum concentration of pancreatic enzymes (amylase and lipase) is commonly an expression of inflammatory or neoplastic pancreatic disease. However, an elevation of pancreatic enzymes, generally mild, may be a non-specific phenomenon without any clinical implication. The large spreading of the serum pancreatic enzymes dosage in clinical practice, particularly in emergency rooms, results in a large number of patients with elevated amylase and/or lipase serum levels without clinical evidence of pancreatic disease [1, 2]. This generally involves a ever-increasing number of instrumental and biochemical investigations to exclude pancreatic disorders, with a waste of resources from a costeffective point of view. This review emphasizes the biological mechanisms behind these serological alterations, the possible causes, the clinical implications and the diagnostic approach.

Healthy Chinese with benign pancreatic hyperenzymemia.
Benign pancreatic hyperenzymemia, or Gullo's syndrome, is an uncommon syndrome characterized by a long-term increase of serum pancreatic enzyme in the absence of pancreatic diseases. It is primarily discovered incidentally and occurs in either sporadic or familial form. Herein, we report the first case of benign pancreatic hyperenzymemia in Taiwan. A 57-year-old Chinese male was incidentally no...
متن کاملLong-Standing Pancreatic Hyperenzymemia: Is It a Nonpathological Condition?
Chronic nonpathological pancreatic hyperenzymemia is characterized by a chronic, abnormal increase in the serum concentrations of the pancreatic enzymes including amylase, pancreatic isoamylase, lipase and trypsin. The diagnostic work-up that the physicians should recommend to subjects with hyperenzymemia to definitively assess this syndrome is still an open question. A 72-year-old female was a...
متن کاملFamilial Pancreatic Hyperenzymemia
Asymptomatic chronic pancreatic hyperenzymemia is characterized by an increase in serum amylase and/or lipase concentrations and was first described by Pieper-Bigelow et al. [1], in 1990 who called the condition ‘‘asymptomatic chronic hyperamylasemia’’; in 1991, Ventrucci et al. [2] found that, in some patients, pancreatic hyperamylasemia may also be associated with hyperlipasemia and, finally,...

Benign pancreatic hyperenzymemia (Gullo syndrome) is characterized by a more than threefold increase of the pancreatic enzymes lipase and amylase in the absence of a pancreatic disease over a period of more than 1 year, with elevations and significant undulations of pancreatic enzyme serum concentrations occurring on a day-to-day basis for 5 consecutive days. Nonspecific abdominal complaints ma...
متن کاملBenign pancreatic hyperenzymemia or Gullo's syndrome.
Benign pancreatic hyperenzymemia is a newly identified syndrome characterized by an abnormal increase in serum pancreatic enzymes in the absence of pancreatic disease. The hyperenzymemia can occur sporadically or in a familial form, and all of the pancreatic enzymes show elevations. Although the condition is persistent, the enzyme elevations fluctuate considerably, even temporarily returning to...
